A new patient has to arrive 15 minutes prior to the appointment and bring the required documents:
Patients below 18 years of age will be required to be accompanied by guardian and present the following documents
- Valid Emirates ID
- Birth Certificate
- Guardian’s Valid Emirates ID
Patients above 18 years of Age will require the following documents
- Valid Emirates ID
Patient from GCC will require the following documents
- Required proof of residency in Umm Al Quwain
- Valid Emirates ID or Proof of residence in the UAE
Gynecology (for pregnancy) require the following documents
- Marriage certificate
- Patient valid Emirates ID
- Husband valid Emirates ID
